Survey on reach of CRUK SACT regimen-specific consent forms

The national regimen specific consent form project team would like you to complete a short survey to help us to  evaluate the uptake of  the regimen specific SACT consent forms in England, Northern Ireland and Wales in comparison to trusts/hospitals using their own forms.

We would like to understand the challenges related to the introduction and continued use of the forms.

This questionnaire is for any clinician involved in the SACT consent process (i.e. doctor, nurse, pharmacist or equivalent) within NHS hospitals/Trusts in the UK.

 

This survey will take no longer than a couple of minutes to complete. Your feedback will be very helpful to us to improve the uptake and quality of the forms, and also the aspirations or successes in electronic consent across the country.
